What Is Fotona 4D?
The Fotona laser 4D treatment is a face-lifting service that combines four laser modes to address both the surface and structural components of facial skin. The technology works in sequential stages to support collagen renewal, improve tone and refine texture.
- SMOOTH®: SMOOTH targets internal oral tissue to gently heat and contract collagen, which helps to lift and contract tissue in the lower face.
- FRAC3®: This mode focuses directly on deep lines and pigmentation for a smoother and more uniform surface.
- PIANO®: During this mode, a hyper-long Nd:YAG laser pulse is directed into layers of skin to tighten the tissue and promote collagen production and remodelling.
- SupErficial™: Provides a light resurfacing effect to smooth the outer layer and refine the skin’s surface.
We can also include options such as SmoothEye® for the periocular region or LipLase® for the lips and surrounding area. Treatments are adaptable, allowing adjustment of parameters and modalities to align with each individual’s skin condition and treatment goals.
We have also added T-runner, which enables us to offer Fotona 5D with Honeycomb. T-runner is a robotic scanner that provides an automated and uniform delivery in a hexagonal beam pattern that enhances consistency during treatment. This upgrade expands what we can offer within the Fotona platform and provides an additional option for patients who may benefit from the Honeycomb modality.
How Does Fotona 4D Work?
Fotona 4D uses two complementary laser wavelengths that interact with the skin at different depths and tissue types. Each wavelength has characteristics that make it suitable for distinct functions within the treatment sequence.
- Nd:YAG: This wavelength penetrates into the dermal structures to deliver controlled thermal energy that stimulates collagen remodelling and supports firmer tissue. It also targets vascular components and promotes a more uniform skin tone.
- Er:YAG: With this laser, we can perform both ablative and nonablative treatments. It treats the surface of the skin and improves the texture and overall appearance with many customisations.

Preparation
Preparation for Fotona 4D is limited, but it is important to let us know about any medications or supplements you are currently taking, as some may need to be adjusted or paused before treatment. Arriving with clean skin free of makeup or skincare products helps make your treatment more streamlined, though we will also cleanse your skin before we begin. Avoid alcohol before and after your appointment to support the skin’s natural recovery process and limit bruising.

Treatment
You will rest comfortably in our treatment chair while we use each technology in turn. The treatment length will depend on how many modes we are using during your treatment and the areas we are treating. Laser treatments can come with sensations like warmth or tingling, but Fotona’s technology reduces discomfort.

What Is the Average Downtime and Recovery?
Downtime is usually short following a 4D Fotona laser treatment, although the degree of redness or mild swelling can vary depending on skin type and treatment intensity. Your skin may feel warm and look flushed for several hours after the procedure, similar to a mild sunburn.
It is usually best to avoid makeup and products that clog the skin until your face has settled and healed from the treatment. You will be able to return to most of your normal activities the same day, but we will have you stay out of direct sunlight and avoid extreme temperatures.
What Results Can I Expect From Fotona 4D?
You can start to see visible improvement in the weeks following your treatment as collagen production and tissue renewal progress. 4D Fotona laser results can also continue to be refined with subsequent sessions.

Fotona 4D laser treatments can feel warm or mildly prickly. The device incorporates CoolMist technology, which helps moderate surface temperature and keep you comfortable throughout the session. Each stage of the treatment is monitored closely, and we adjust the parameters according to how you respond.
Temporary redness, mild swelling and a sensation of tightness are common reactions following treatment. The skin can occasionally develop light flaking or dryness as it renews, particularly after the SupErficial stage of the protocol. Slight bruising or uneven pigmentation may occur, especially if the skin has been recently exposed to the sun or active skincare products. Following all pre- and post-treatment guidelines helps reduce the likelihood of adverse responses.
Treatment frequency depends on your skin condition, goals, and how your skin responds to laser stimulation. You may benefit from a series of sessions spaced several weeks apart. Maintenance sessions can then be scheduled periodically to support collagen renewal and sustain results. Dr Aleks or Dr Indi will recommend a schedule that works for you.

Pricing for the Laser 4D Fotona treatment depends on the treatment area, the number of sessions required and whether procedures such as SmoothEye or LipLase are included.
